ABSTRACT

Background: Foodborne diseases have increased considerably. Aim: To report a foodborne outbreak, remarking the importance of early notification to activate the epidemiological surveillance system. Results: During February 2011 we observed a Salmonella Enteritidis outbreak. 31.6% of the cases were seen in the same Emergency Care Unit where all required intravenous fluid rehydration, and 41.7% were hospitalized because of severe dehydration. In the Emergency Room 45.5% of cases required a second visit to be diagnosed correctly. Discussion: Physicians under report the cases of this disease, delaying the activation of the epidemiological surveillance system. Conclusions: Besides providing good treatment to patients, physicians need to be qualified to recognize foodborne diseases and communicate early the suspicion of an outbreak to the epidemiological surveillance system in order to prevent new cases of disease in the community.


Introducción: Las enfermedades transmitidas por alimentos (ETA) han aumentado considerablemente. Objetivo: Reportar un grave brote de ETA destacando la importancia de la notificación precoz para la activación del sistema de vigilancia epidemiológica. Resultados: Durante febrero de 2011 se observó un brote de Salmonella Enteritidis. Un 31,6% de los casos fueron atendidos en un mismo servicio de urgencia, donde todos requirieron la administración de fluidos endovenosos y 41,7% fueron hospitalizados por deshidratación grave. El 45,5% de los casos necesitó de una segunda consulta para ser diagnosticados correctamente. Discusión: La identificación de pacientes integrantes de un brote de ETA es difícil en los servicios de urgencia y los médicos sub-reportan los casos, retrasando al sistema de vigilancia epidemiológica. Conclusiones: Junto con brindar un adecuado tratamiento, los médicos deben estar capacitados para reconocer las ETA y comunicar tempranamente la sospecha de un brote para prevenir nuevos casos en la comunidad.

Cardiac involvement with Salmonella infection is well known and electrocardiographic changes bear a direct relationship to prognosis. We present here a case with ECG changes mimicking an acute myocardial infarction in salmonella infection.

ABSTRACT

Se presenta el caso de un paciente masculino de 59 años de edad, con antecedentes de colitis ulcerosa, que luego de la ingesta de agua no potable se internó por diarrea de una semana de evolución con más de veinte deposiciones acuosas diarias. Al momento de la internación se realizó rectosigmoidoscopía que no reveló actividad de su enfermedad intestinal de base. Se solicitaron estudios para descartar diversas etiologías infecciosas, obteniéndose en coprocultivo desarrollo de Salmonella species por lo que se indicó tratamiento antibiótico con ciprofloxacina.

Las diarreas causadas por bacterias son las que potencilamente suponene un mayor riesgo vital para el paciente, siendo los patógenos más importantes Shiggella y Salmonella que se ubican entre las causas principales de muerte en niños menores de cinco años, e spor esto que resulta de vital importancia realizar un diagnóstico rápido y preciso, junto a un tratamiento efectivo.

ABSTRACT

Background: Salmonella enteritidis (SE) is a frequent cause of diarrhea, and is transmitted mainly by SE contaminated eggs or poultry meat. The frequency of SE contaminated eggs or chicken meat and the risk for acquiring this pathogen is unknown in Chile. Aim: To measure SE contamination in eggs poultry meat and entrails offered in retail markets in the Metropolitan Area during two consecutive years (1998-1999). Material and methods: Samples were placed in sterile bags and transported to the laboratory before 4 hours at 4­C. Microbiologic detection was done using a standard procedure and an immunodetection assay. Results: SE was found in one of 1081 egg samples (0.09 percent). The contaminated sample was offered in a supermarket under their own commercial name. Six percent of 1154 poultry meat samples were contaminated by SE and 2.3 percent by other Salmonella serotypes. Entrails had even higher rates with 10.2 percent of 370 samples harboring SE and 2,7 percent other serotypes. Total Salmonella sp. isolates and SE isolates declined during 1999. Nine SE phagotypes were identified, predominating types 4 and 7. Conclusions: Eggs and other avian products are contaminated by different SE phagotypes and other Salmonella serotypes, implicating a risk for the consumers

ABSTRACT

Um aumento súbito de isolamentos de salmonelas, identificados como de Salmonella enteritidis soro grupo D, foi observado. As amostras foram isoladas em cultura de fezes (coprocultura) e sangue (hemocultura). A maioria dos pacientes e entrevistados revelou ingestäo ingestäo de alimentos preparados com produtos e subprodutos à base de ovos, aves e seus derivados. As cepas obtidas se mostraram sensíveis a: ampicilina, cefalotina, ceftriaxona, sulfametoxazol/trimetoprima, amicacina e gentamicina. Uma permanente vigilância das salmoneloses é imperativa, haja vista a persistência de isolamentos de Salmonella enteritidis em casos esporádicos e em surto
